Registration Requirements
Your child must be 5 years of age on or before September 1, 2007
THE FOLLOWING DOCUMENTS MUST BE PROVIDED:
Certified (long form) copy of your child’s birth certificate**
An updated Immunization Certificate of Compliance
(Form #121 or a Temporary Compliance Form #123 from the Health Dept.)
Your child’s Social Security card
Two (2) proofs of residency (mandated by the Mississippi Legislature)
Examples: apartment or house lease
Filed homestead exemption application form
Mortgage document or property deed
One utility bill
Driver’s license
Voter precinct identification
Other documentation that unequivocally proves residence
Your child must accompany you.
Please bring any hearing or visual aids your child regularly uses. You will be asked to complete registration forms and parent observation forms while your child is involved in the vision/hearing screening process.
Classrooms will not be open for visitation; however, you will be invited to attend an open house at the beginning of the school year.
Please allow a minimum of one hour to complete the registration process.
